PERSONAL INFORMATION COLLECTION STATEMENT

This is a personal information collection statement (“PICS”) given in compliance with the requirements of the Personal Data (Privacy) Ordinance (“PDPO”) (Cap. 486 of the Laws of Hong Kong). Nothing in this PICS shall limit your rights under the PDPO.

  1. Types of Personal Data

    Personal data collected by Academy of Oral and Maxillofacial Implantologists Limited (“Academy”, “we”, “us” or “our”) from you include:


    1. identification and personal information such as your name, date of birth, gender, nationality, identity card/passport details and photographs;
    2. contact details such as your residential or business address, telephone number, e-mail address, social media account and emergency contact;
    3. financial details such as your bank account and/or credit card number;
    4. evaluative data such as your academic records, qualifications, membership of professional associations and work experience; and
    5. information collected via cookies when you access our website or any online applications such as webpages visited by you, options selected and links clicked.

     

  2. Purpose of Collection

    The personal data provided by you to us will be used for the following purposes:

    1. processing any application by you to become a member of Academy in any category and payments by you of subscriptions and any other monies due to us;
    2. arranging for, or inviting, you to participate in any activities arranged by Academy, alone or with other organisations;
    3. verifying your academic and professional qualifications, status and experience, in connection with, inter alia, your application to become a Fellow or an Ordinary Member of Academy;
    4. conducting any research, surveys and/or review of matters relating to Academy and its activities;
    5. assisting Academy to comply with all applicable laws, regulations or court orders (whether in Hong Kong or any other place), any request by a governmental, statutory, regulatory or law enforcement authority (whether in Hong Kong or any other place);
    6. facilitating communications between you and Academy;
    7. subject to no objection, for Academy to send you marketing materials about upcoming events and courses, new products or services, newsletters and other benefits relating to your membership of Academy; and
    8. subject to your prior written consent, for direct marketing by Academy’s partners of such upcoming events and courses, new products or services, newsletters or such other benefits.

  3. Voluntary Provision of Personal Data

    You are not legally obliged to provide any personal data to Academy, but failure to do so may mean that we cannot process any application for membership or to participate in the activities of Academy and we may be unable to maintain contact with you.

  4. Disclosure and Transfer of Personal Data

    Academy will keep your personal data confidential. Only persons expressly authorised by Academy will have access to, and handle, your personal data. Academy may disclose or transfer your personal data to the following parties, either within or outside Hong Kong, solely for the purposes set out in paragraph 2. above:

    1. organisers of educational and research functions, whether or not jointly with Academy, relating to implant dentistry and maxillofacial surgery including clinical trials, seminars, symposia and lectures provided that Academy is reasonably satisfied that such functions may be beneficial to you and other members; and
    2. any agency, professional service provider or other contractor engaged by Academy to provide administrative or other services provided that any such organiser, agency, professional service provider or contractor is under a duty of confidentiality to Academy and obliged to keep your personal data confidential.


    We may also disclose your personal data when authorised or required by law and in response to requests from governmental, statutory, regulatory or law enforcement authorities.

     

  5. Use and Provision of Personal Data in Direct Marketing

    Please note the following:

    1. subject to no objection, Academy may use your personal data to provide you with promotional materials about (i) upcoming professional events, seminars and courses; (ii) facilities and services relating to oral and maxillofacial surgery and implantology; (iii) supplies of related goods and equipment; and (iv) special offers or other benefits relating to your membership of the Academy; and
    2. subject to your prior written consent and compliance with PDPO, Academy may also disclose or transfer your personal data to any third party including (a) any member of the Straumann group of companies; or (b) any distributor of any such member, in the country of your domicile, for the purpose of any such third party providing you with promotional materials of the types described in (i) to (iv) above.


    Personal data to be used by AOMI and/or any such third parties for the purposes of direct marketing shall comprise only your name and contact details (such as e-mail address and/or mobile telephone number).

    Please note that the consent given by you for use of your personal data in direct marketing may be withdrawn by you at any time, by notification to Academy’s Personal Data (Privacy) Officer (see Section 6. below).

  6. Access to and Correction of Your Personal Data

    You have the right to request access to, and correction of, your personal data held by us by completing a data access form, available from our Personal Data (Privacy) Officer. A fee may be imposed by us for processing any such request. Normally, we will respond to any such request within 40 calendar days of the date of submission of the request.

    Any personal data access or correction request should be made by completing such form and sending it to our Personal Data (Privacy) Officer at Suite A, 16/F, Tesbury Ctr, 24 – 32 Queen’s Rd East, Hong Kong, Hong Kong/e-mail dpo@aomi.org.

  7. Amendments

    This PICS is subject to review and may be amended from time to time. You may contact our Personal Data (Privacy) Officer or visit our website (www.aomi.org) for the latest version.
